I need to know how you want your name printed on your certificate. I send the certificates to a print shop to be professionally printed prior to the 1st day of class. The printer needs time to complete the order and I need time to pick them up. **If I don't receive the completed form at least 24 hours prior to the start of class, I won't have your certificate to give to you at the end of class. So, kindly complete this form asap. Thanks! :)
Contact phone number: I also need a contact phone number in case of emergency or if I need to contact you urgently regarding the class. Don't worry...I'll only contact you via phone for emergency/urgent matters regarding class.
